Additional comments

If your Adoption Application is approved, it will be necessary for you to have an initial telephone interview with one of our Adoption Committee members.  We will also schedule a Home Visit prior to completion of an Adoption.  When is a convenient time for you to schedule the initial interview?
NOTE:
If we approve an adoption of an OES who is too young to spay/neuter, placement will be handled as a Foster Home until the OES is old enough to have the surgery.  Once proof of surgery is submitted, we will then complete the final Adoption paperwork.

Submission of your application
does NOT guarantee that you will be
approved to adopt one of our rescued OES.

By signing this application, you acknowledge that you have read our Terms and Conditions of Adoption, that you understand and agree to the Terms, and will abide by our regulations therein.
